Subacute Care

The goals of subacute care are medical recovery and stabilization, increased independence and making the transition home. Optimal progress is what MCNR is all about.

MCNR serves adults of all ages needing treatment related to:

Accident or injury
Orthopedic surgery
Joint replacement
Post-surgical rehabilitation
Stroke
Complex wounds
Chronic Obstructive Pulmonary Disease (COPD)
Other pulmonary conditions
Pain management
Palliative care
Hospice care

The subacute care wing has a dedicated staff that works intensively with individuals to help them achieve their goals.
Our interdisciplinary team develops and implements a collaborative care plan, customized for each patient, and coordinated with his or her physician. The components of the plan are:

Time: Usually short-term, ranging in length from a few days up to three months.
Functional Gains: Our primary goal is helping patients increase their level of daily function and independence as quickly and safely as possible.
Expectations following completion of program: Our program is designed to prepare both the patient and family for the most effective and appropriate setting.
